OUR STREAMLINED REFERRAL PATHWAY
Guided Journey Outreach Services offers several simple ways to make a referral, including phone, email, or our secure online referral form. Referrals may come directly from individuals and families or from insurance companies and case managers supporting clients who are seeking mental health services.
We understand that mental health and behavioral concerns often need timely attention. Once a referral is received, our team reaches out to the client or family within 24 to 48 business hours to begin the intake process. Our goal is to respond with care, minimize wait times, and help ensure support begins as smoothly as possible.
Thorough clinical assessment exploring the individual’s and family’s strengths, challenges, and needs, providing a clear foundation for thoughtful, personalized treatment planning.
Collaborative creation of a personalized service plan, creating meaningful goals and tailored strategies with clear steps and timelines to support the individual’s growth and well-being.
Detailed evaluation using CANS/ANSA to identify the individual’s strengths and areas of need, supporting a holistic understanding that informs personalized, strengths-based planning.
Flexible Services to Meet Your Needs
We know that every family’s situation looks different, so we offer flexible service options designed to fit into your real life. Services are traditionally provided in person to support a strong connection and engagement. When it is more convenient for you, your family, or to better meet client needs, telehealth options are available. We thoughtfully match families with counselors based on clinical needs, personality fit, and cultural considerations, so support feels comfortable, respectful, and effective.
Scheduling is built around family priorities whenever possible, including after-school sessions and limited weekend availability depending on counselor schedules. With appropriate district authorization, services may also be provided within school settings. For children who feel most at ease outside of an office, services can take place in the home or community, including outdoor or familiar environments. Our goal is to remove barriers, reduce stress, and meet children and families where they are most comfortable.
PROCESS, PARTNERSHIP, & PROGRESS
After intake, a compassionate counselor will be assigned to the case and will reach out directly to schedule an initial appointment at a time that works for you. Early sessions focus on building rapport, understanding priorities, and creating a consistent schedule that fits your family’s needs while allowing flexibility as things change.
Caregivers are viewed as essential partners in the therapeutic process. Counselors maintain open communication with families, offering guidance, education, and support. Our team includes counselors certified in Nurturing Parenting programs, allowing skills and strategies to be reinforced within the home and caregivers.
Services are provided with respect for each family’s culture, values, and lived experience. Counselor matching takes into account clinical needs, personal fit, and cultural considerations. Our team remains responsive to feedback and changing needs, creating a supportive environment where families feel heard, respected, and included.
With consent, counselors collaborate with schools, doctors, case managers, and other services to ensure care is aligned. During intake, communication is coordinated with school teams, primary care providers, and any additional behavioral health professionals to help create consistency across settings.
Progress is reviewed regularly, with quarterly reports, to ensure services remain meaningful and individually tailored. Goals and strategies are adjusted as needs evolve, strengths emerge, or new challenges arise, allowing care to remain flexible and aligned with the individual’s growth
over time.
When additional support is needed, counselors assist families in connecting to outside services. This may include medication management, new medical providers, higher levels of care, psychological evaluations, ABA therapy, and resources such as food pantries, employment support, homeschooling assistance, and other local services.
We are proud to be in-network with a variety of Medicaid insurance providers and are always looking to expand our network. We welcome representatives to reach out to explore partnerships that can help bring more accessible mental health services to underserved communities in the DFW area and beyond.
