Complete our referral with signed relaeses of information to primary care doctor and school if applicable to confirm mental health diagnosis, emotional needs, and behavioral challenges.
Clients must be able to speak English and/or Spanish, as services are currently only offered in these two languages.
Telehealth options can be made available for appropriate services at the request of the client and their family.
Skills Training is a goal-directed service designed to help individuals build, restore, and strengthen the skills needed to live safely, independently, and successfully within their community. Services focus on supporting individuals in returning to their highest level of baseline functioning while promoting long-term stability in the most appropriate, least restrictive environment.
Skills Training is provided through face-to-face activities, instruction, and individualized interventions. All services are guided by the Individual Service Plan and are tailored to support meaningful, real-life skill development.
Areas of focus may include:
Functional skills and appropriate behaviors related to health and safety
Instrumental activities of daily living and effective use of the community
Education and monitoring related to mental health, nutrition, and physical well-being
Identifying and using community resources
Organization, structure, and daily routines
Anger management and emotional regulation
Development and enhancement of social and interpersonal skills
Targeted Case Management (TCM) supports individual and family wellness through education, communication, service coordination, and referrals. This time-limited service provides a structured, organized approach to help individuals move through meaningful change toward greater independence and self-sufficiency.
TCM focuses on connecting eligible individuals to the medical, social, educational, and community-based services they need to thrive. The process is built on collaboration and shared decision-making, recognizing that lasting progress happens when clients are active participants in their care.
Targeted Case Management is a partnership involving the individual, their family or support system, community resources, and the case manager. Clients and families are engaged throughout every phase of the process, including assessment, planning, problem-solving, and identifying appropriate resources.
Through ongoing coordination and support, TCM helps ensure that available resources are accessed efficiently and used effectively.
Crisis Intervention is a short-term, immediate support service designed to help individuals and families navigate overwhelming situations that feel unmanageable. A crisis may involve emotional distress, behavioral concerns, or sudden changes that place an individual’s safety, stability, or well-being at risk.
Crisis intervention involves three key components:
The crisis itself, or the individual’s perception of an unmanageable situation
The individual or family experiencing the crisis
A trained case manager who provides timely, skilled support
The goal of crisis intervention is to stabilize the situation and reduce the risk of further emotional or physical deterioration. Services focus on helping individuals cope effectively, regain a sense of control, and identify immediate supports and next steps.
Intervention is responsive and supportive, emphasizing safety, problem-solving, and connection to appropriate resources. When needed, families and support systems are included to help strengthen coping strategies and promote ongoing stability beyond the immediate crisis.
Medication Training and Support services are provided by a prescribing professional to help reduce symptoms associated with a mental health disorder. These services focus not only on medication management, but also on education and ongoing support for the individual and their care team.
In addition to prescribing and monitoring medication, the provider works closely with the client and case manager to increase understanding of the individual’s mental health diagnosis and treatment plan. Education is tailored to the client’s needs and helps ensure medications are used safely and effectively.
